The Family Doctors (Shreveport, Louisiana, U.S.A.) uses data from its Allscripts Professional EHR™ suite to help improve care for patients with chronic disease. Blue Cross Blue Shield of Louisiana recognized The Family Doctors and its physicians with 26 awards in 2015 for highest achievement in population health management. Improving care for a community that struggles with chronic disease The Family Doctors serves a community facing significant health concerns. The State of Louisiana has some of the highest healthcare costs and lowest quality outcomes in the United States. According to America’s Health Rankings® of the 50 states, Louisiana has not ranked above 47th in more than 20 years. Chronic diseases are especially costly, as Louisiana has one of the highest rates of cardiovascular and diabetes mortality rates. Working closely with the state’s largest payer, Blue Cross Blue Shield of Louisiana, the […]

Every year, clinicians manually enter billions of point-of-care test results (such as urinalysis and cholesterol tests), from paper printouts into electronic health records (EHR). This is a time-consuming process, as a single test result can contain a dozen different numerical values and units. Each data point is an opportunity for error. To solve this problem one of Allscripts Developer Program partners, Relaymed, researched the current methods used by clinicians and incorporated this information as part of its user-centered design process to create its own automated workflow. Unfortunately, when Relaymed researchers evaluated the manual methods for entering test results into the EHR at multiple organizations, they found it doesn’t always happen successfully. Relaymed Chief Executive Officer Neil Farish reports that his team of researchers found numerous issues, including a continuous printout of lab results left hanging from a device for more […]

Studies show that some physicians are spending more than 40% of their time on patient notes, leading many providers to feel like they’re spending less time with patients and more time with patient documentation. Several alternatives try and address this issue, from creating templates that enable a clinician to easily document common problems, to hiring scribes to handle data entry while the physician is examining the patient. Each method has its advantages and offers some benefits, but Allscripts Developer Program partner NoteSwift offers an alternative solution that uses voice direction to help providers move quickly through the patient chart and document or place orders with speed and accuracy. Developed by a neurologist who felt the pain and tedium of patient documentation in his own practice, NoteSwift enhances productivity for providers in every specialty so they can see more patients and […]

4 generations of medical innovation

Founded in 1911, Dominion Medical Associates (Richmond, Virginia, U.S.A.) has a long history of delivering excellent health care to its patients. In a recent case study, Director of Clinical Research Richard “AJ” Jackson Jr., MBA, CRC said, “My great-grandfather founded our practice, and through the generations we’ve been grateful to be able to maintain our independence and give back to the community.” Small but mighty With three physicians and one mid-level provider, the practice serves more than 20,000 active patients across two locations. Dominion Medical Associates uses Allscripts Professional™ Suite to participate in many innovative initiatives, typically only taken on by larger practices. For example: Value-based-care payment models – As health care shifts from fee-for-service to value-based-care payment models, Dominion Medical Associates can use existing data to adapt. It joined an Accountable Care Organization (ACO) in 2014. It is also […]

Because our clients consistently innovate and exchange best practices, they represent the best and brightest in health care. One of the ways we help fuel their progress is by staying on top of industry trends and highlighting best practices. To further help the healthcare community succeed, we’re sharing these insights and client outcomes through our new magazine: Continuum. We launched our first issue, and some of the highlights include: Exchanging patient information to improve care. Canadian client Fraser Health Authority uses the Allscripts dbMotion™Solution to make all relevant clinical information from multiple source systems available at the point of care. Is ACO the Way to Go? What independent physician practices should consider reimbursement models shift to value-based-care models, such as Accountable Care Organizations (ACOs). Focusing on the Patient. Family Practice Associates, a large family practice in central Kentucky, U.S.A., describes how Allscripts Professional […]

To compete in today’s value-based-care market, many physician practices give up their independence. Research commissioned by The Physicians Foundation found that only 35% of physicians describe themselves as independent practice owners, down from 49% in 2012 and 62% in 2008. But we find a different story at Family Practice Associates, one of the largest independent family practice groups in central Kentucky. How this practice remains competitive – while maintaining its independence – is the subject of a new video: The integrated suite of Allscripts Professional EHRTM, Allscripts Practice ManagementTM and Allscripts FollowMyHealth® patient portal helped the group earn $375,000 in Meaningful Use incentive payments, improve net collections to 98%, and enroll 20% of its patients in the portal in less than a year. “Family Practice Associates has always been a very independent organization and wants to remain that way,” Administrator […]

Finding tomorrow’s high-risk patients

As the healthcare industry offers more ways to earn risk-based or value-based revenue, providers are more proactively coordinating care, engaging patients and managing population health. Analytics are fundamental to all of these strategies, even in small practices. To be successful, physician practices need analytics that are predictive across multiple populations. They need to know who the high-risk patients are today, but they also need to identify the “rising-risk” patients. With intervention, these patients can have a healthier future. The cost of unmanaged cardiac conditions One healthcare organization evaluated its Accountable Care Organization (ACO) population with Johns Hopkins Adjusted Clinical Groups (ACG®), an industry standard to adjust risk and predict future healthcare usage. With analytics, this ACO found that more than half of its patients were either high risk (30%) or very high risk (27%). Furthermore, it found that the average […]

Best client stories of 2014

I’ve always admired the people who work in health care. They’re even more on my mind this month with the recent passing of my father. The outpouring of support I received made me think about the many other people whose loved ones need medical care during this holiday season. Our clients care for thousands of people every day who mean a lot to their families. As 2014 draws to a close, I’d like to take a moment to thank those clients for their efforts to care for others. To honor their work, here some of the client stories that appealed most to readers this year: Alberta Health Services (Calgary, Alberta, Canada) – The organization used Allscripts SunriseTM to build strategic clinical decision support tools that improved clinical results for diabetes management and saved C$600,000 with an order set for a […]
